Nashville Swim League City Meet Sign Up Form

Form and Fees due Tuesday, June 30th.
Fees should be made out to Cottonwood Swim Team.
Place fees in Coke folder at the pool or drop off at the Coke house at
703 Mockingbird Drive.

Fees are $19 per swimmer age 7 and up and $14.50 per swimmer age 6 & under.

Will your swimmer attend City Meet?

Indicate event preference.  Select up to 4 individual events not including relays.  6 & Unders may swim Freestyle, Back Stroke, and the mini IM.  The coaches will place the swimmer in the relays.  You may also select Coaches Preference and the coaches will place the swimmer in the events they feel the swimmer will best perform.
