2011 Nashville Swim League City Meet Sign Up Form

Reminder:  Form and Fees due FRIDAY JUNE 30th
Fees should be made out to Cottonwood Swim Team.
Drop off at the Armstrong's at 181 Riverwood.

Fees: (You may have already paid this fee at registration. We will contact you if you owe City Meet fees.)
$20 per swimmer age 7 and up and
$15 per swimmer age 6 & under.

Indicate event preference.  Select up to 4 individual events not including relays.  6 & Unders may swim Freestyle and Back Stroke. No mini-IMs this year. The coaches will place the swimmer in the relays. You may also select Coaches Preference and the coaches will place the swimmer in the events they feel the swimmer will best perform.
