The completion of this form replaces the Pink Card you received with Consultant Assignment Packet.
By submitting this form, you certify that your JA class has been scheduled. It is important that you submit this form to the Junior Achievement office in order for your volunteerism to be recognized nationally by JA. We request that you submit one Class Start Form for each class in which you teach the Junior Achievement program. You will receive one entry per form in the prize drawing. To be eligible for a prize drawing, Class Start Forms must be received within one week of assignment date and program must be completed.
