It is required that you submit this form to the Junior Achievement office within one week of completion. This certifies that your Junior Achievement class is completed. You must return one Class Completion Form for each class in which you hosted a Junior Achievement program. You will receive one entry per form in the prize drawing. To be eligible for a prize drawing, Class Completion Forms must be received within one week of completion date and program must be completed. Only programs in which the volunteer made the required number of visits will be considered complete.
